Home-Based Programming
First Home Care's home-based services offer intensive behavioral health intervention for children, adolescents, adults and families. Our home-based services are strength-based, family-focused and solutions-oriented.
The goal is to identify, implement and employ sustainable, naturally occurring community support systems, thereby reducing and eventually eliminating the need for ongoing formal system supports. These services include crisis intervention and assessment, supportive counseling, group counseling, medication monitoring, mentoring, parent education, comprehensive discharge planning, and other services designed to address the individual needs of clients and families served.
