First Home Care believes in working to enable children, adolescents, families and adults to receive the care they need — in their homes and communities.
School-Based Programming
School-Based Programming serves youth who are transitioning to or from intensive services. Our case managers work with the youth, family, referring agency and providers to seek the least restrictive and most appropriate environment. We draw on each youth's individual strengths as well as assessment data to determine appropriate placement.
